Supply & Donation Team

Will coordinate donations and gathering of supplies; responsible for procuring commonly needed supplies regularly and specifically-requested supplies on request from a case manager and providing a location at which a member of the driver team will pick up. 


Service Agency Team

Responsible for establishing lines of communication with service agencies and local organizations to send referrals to a Case Manager and send supply requests to the Supply Team.

Case Management Team

Connect individuals with agencies and individuals/agencies with supplies; point people for coordination. Will receive referrals from the Service Agency Team, connect with Supply Team to secure needs, and then activate a member of the Driver Team to make the delivery. May also in the future send referrals to a caregiver team, a meal prep team, or a service team.

Delivery and Drivers Team

Responsible for picking up supplies from the providers upon activation from a case manager and delivering them to the recipient.
